Roots Physical Therapy + Yoga, LLC (“Roots,” “we,” “us”) respects your privacy. This policy explains what information we collect when you visit rootsspecialtyservices.com, how we use it, and the choices you have.

This policy applies to our website only. Health information you share as a patient, including your evaluation, treatment notes and billing records, is protected under the Health Insurance Portability and Accountability Act (HIPAA) and described in our Notice of Privacy Practices [link]. If the two ever differ, the Notice of Privacy Practices governs your health information.

Information we collect

Information you give us. When you fill out our contact form, we collect your first and last name, email address, phone number and the message you write. When you buy a gift card, we collect your name and email, and the recipient’s name and email if you send it to someone else.

Please don’t send health details through the contact form. The form is for general questions. Share medical information by phone or at your appointment, where it is protected under HIPAA.

Booking and payments. Appointments, class sign-ups and gift card purchases are handled by Jane, our scheduling and payment provider, at rootspt.janeapp.com. Information you enter there is collected by Jane and governed by Jane’s privacy policy and our Notice of Privacy Practices. We never see or store your full card number.

Information collected automatically. Like most websites, ours automatically receives basic technical information when you visit, such as your browser type, device, IP address, the pages you view and how you arrived. This comes from our web host, Google Analytics and the cookies described below.

How we use your information

We use the information we collect to:

  • Answer your questions and follow up on your requests
  • Schedule consultations, appointments and classes
  • Process and deliver gift cards
  • Send updates about classes and services, if you’ve asked to receive them [delete if no email list]
  • Understand how visitors use our site so we can improve it
  • Protect our site from spam and misuse

How we share your information

We do not sell or rent your personal information. We share it only with service providers who help us run our practice and website, and only as needed for them to do that work. These include:

  • Jane for scheduling, payments and gift cards
  • WP Engine for website hosting
  • Google for website analytics, maps and spam protection [confirm]

We may also disclose information if required by law, to protect the safety of our clients or staff, or as part of a sale or transfer of the practice.

Cookies and analytics

Cookies are small files your browser stores to make websites work and to measure how they’re used. We use Google Analytics to see, in aggregate, which pages people visit and how they find us. Google Analytics does not tell us who you are. You can learn how Google uses this data at policies.google.com/technologies/partner-sites, and opt out with the Google Analytics opt-out browser add-on.

Our Contact page shows an embedded Google map, and our site may load fonts from Google. Google may set its own cookies when these load. You can block or delete cookies in your browser settings; the site will still work, though some features may not.

Our site links to Jane, Instagram, Facebook, LinkedIn and other websites. We aren’t responsible for their privacy practices, so please review their policies when you visit them.

How we protect your information

Our site uses an encrypted (HTTPS) connection, and we limit access to the information you send us to staff who need it. No website or email is completely secure, so please don’t send sensitive health or financial information through our contact form or by email.

How long we keep it

We keep contact form messages and gift card records only as long as needed to respond to you, provide our services and meet our legal and accounting obligations. Patient records are kept as required by Rhode Island law and HIPAA.

Children’s privacy

Our website is not directed to children under 13, and we do not knowingly collect their information online. If you believe a child has sent us personal information, contact us and we will delete it. Care for minors is arranged by a parent or guardian.

Your choices

You can ask us to access, correct or delete the personal information you’ve given us through this website, or to stop sending you emails, by contacting us below. You can unsubscribe from any marketing email using the link at the bottom of the message.

Changes to this policy

We may update this policy from time to time. When we do, we’ll post the new version here and change the effective date at the top.
